4. Place your two duplicate receipts, and your additional prescription for Eldepryl® (selegiline hydrochloride) Capsules (make certain to send the actual prescription given to you by your doctor, and not a photocopy) in the pocket of the completed self-mailer, and drop it in the mail.
Important note: To ensure timely receipt of your free bottle, complete step 4 immediately after you fill your second month's prescription obtain your duplicate receipt.
It's that easy! A mail-order pharmacy will receive all of your information, and in just 2 to 3 weeks your free 1-month supply of Eldepryl Capsules will arrive, along with the materials you need to participate again.
